Support Services

We provide education and support services to build a healthier Kansas City.

Our health insurance experts assist with  Medicare, Medicaid, and Marketplace (also known as the Affordable Care Act or Obamacare) insurance. We help individuals and families understand the benefits and costs of the plans that are available to them.  Our services are free and unbiased. Once you are enrolled, we’ll help you understand how to use your insurance to get the care you need and stay healthy. Contact us to at 816-753-5144 or navigators@kccare.org to learn more.

Community Health Workers (CHWs) are community residents trained to help people navigate the complex health system.  Through a powerful peer relationship, a CHW can help you identify your biggest health challenges and create a plan to overcome those challenges. CHWs partner with clinics, hospitals, and communities to assist patients. Contact us at bryanc@kccare.org to learn more about partnering or make a referral for services.

KC CARE Health Center provides opt-out testing for our patients, and we help other healthcare providers implement this model. We provide training and ongoing support to help make sure HIV+ persons in our community are connected with the care they need. Contact us at jessicam@kccare.org to learn more about using this model at your practice.

Through Linkage to Care, HIV+ persons receive focused case management as they connect to HIV care and ongoing treatment. This service is for those who are newly diagnosed, new to the area, or have fallen out of care. We accept referrals from the entire community to make sure all HIV+ persons get the care they need. Contact the Linkage pager number (816-990-2411) for more information.
